For 2011 Reports, please use the on-line report creation form linked below. Wireless Service Providers may submit their reports via e-mail. Competitive Telecommunications Service Providers and Incumbent Local Exchange Carriers should file their reports in the Cases listed below:

All regulated telecommunications companies who have not yet subscribed to the telephone industry listserve should do so by going to the PUCO Docketing page (http://www.puco.ohio.gov/puco/docketing), clicking on Industry Listserve Sign Up, and selecting the telephone industry service list. Companies should subscribe multiple addresses and take appropriate actions to ensure that electronic mail service from the Commission is not blocked or disabled.
