Annual Report On-line Form and Filing Instructions
for Natural Gas Distribution Companies, Certified Competitive Retail Electric Service and Natural Gas Suppliers, Governmental Aggregators, Competitive Telecommunications Service Providers, and Incumbent Local Exchange Carriers
(Wireless Telecommunications Service Providers see separate instructions. All other industry segments see 2011 Filing Instructions).
If you electronically filed your Annual Report last year, use your same user name and password to file this year and skip to Step B.
A. For filers who do NOT already have a PUCO account (a username and password) and E-filing profile:
- Submit an electronic filing participation agreement
- Create a PUCO account (username and password)
- Create an e-filing profile
- Send an email message to annualreports@puc.state.oh.us with “Annual Report On-line Form” in the subject line, and listing the following information in the text of the message:
Your PUCO username
Each company you will be filing on behalf of
Your name
Your phone number
B. For filers who already have a PUCO account and E-filing profile:
The filing process for all Natural Gas Distribution Companies, Certified Competitive Retail Electric Service and Natural Gas Suppliers, Governmental Aggregators, Competitive Telecommunications Service Providers, and Incumbent Local Exchange Carriers consists of the following 3 steps (Wireless Telecommunications Service Providers: see separate instructions).
- Complete the appropriate industry on-line form and save the PDF file of the report to your hard drive.
- Log in to PUCO’s Docketing Information System (DIS) and e-file your completed report in the appropriate case before April 30, 2012 at 5:30 p.m. You will need to upload the PDF file created in step 1. Print and save a copy of the confirmation page showing that you have successfully filed your Annual Report. Do NOT file a hard copy of your report.
- Either email an electronic copy to annualreports@occ.state.oh.us or send a paper copy to:

Beginning this year, any regulated entity that wishes to continue receiving paper copy service from the Commission via regular mail, must submit a request form. All other regulated entities should subscribe to the appropriate industry listserve(s) using multiple addresses and take appropriate actions to ensure that electronic mail service from the Commission is not blocked or disabled.
