2010 On-line Assessment Report Form Instructions for Wireless Telecommunications Service Providers
For 2010 Reports, an on-line assessment report creation form is now available for all Wireless Telecommunications Service Providers.
In Case No. 09-01-AU-UNC, the Commission established processes for the electronic filing of annual reports using the PUCO’s Docketing Information System, to reduce the costs and increase the efficiency of processing these reports for both the filers and the Commission. The Commission expects all reporting companies to cooperate in the transition to a paperless filing process by subscribing to the appropriate electronic mail industry listserve(s), and filing their reports electronically, or contacting the Docketing Division at (614) 466-4095 if they are unable to do so. Reporting companies should subscribe multiple addresses and take appropriate actions to ensure that electronic mail service from the Commission is not blocked or disabled.
The process for all Wireless Telecommunications Service Providers consists of (1) completing the on-line web form, (2) electronically mailing the completed report to annualreports@puc.state.oh.us, and (3) sending a paper copy to the Ohio Consumers’ Counsel or emailing an electronic copy to annualreports@occ.state.oh.us. Do NOT send a hard copy of your report if you have successfully sent your report electronically.
If you don't already have one, you will need a PUCO account (a username and password, which is also used for electronic filing) to complete the on-line assessment form. You can create a PUCO account by going to the PUCO Docketing page and clicking on Get a NEW PUCO Account.
Step 1:
Once you have a PUCO account, send an email message to annualreports@puc.state.oh.us with “Annual Assessment On-line Form” in the subject line, and listing the following information in the text of the message:
- Your PUCO username
- Each company you will be filing on behalf of
- Your name
- Your phone number
You will be sent instructions for completing the on-line annual report form.
Step 2:
Complete the on-line report form and create a PDF (portable document format) file, including the signed affidavit of an authorized company officer, and save it to your computer.
Step 3:
Send the completed PDF file via e-mail to annualreports@puc.state.oh.us before May 2, 2010 at 5:30 p.m. If you have successfully e-mailed your report, do not send a hard copy to the Commission.
Step 4:
Send the completed PDF file to annualreports@occ.state.oh.us or a paper copy to:
