Cases filed with the Public Utilities Commission of Ohio are identified by a four element coding system. The coding elements are: the year in which the case was filed, the sequence in the filing, the industry code, and the purpose code. The industry code indicates the utility involved in the case and the purpose code indicates the reason for which the case has been brought before the Commission. For example, a water company applying for a certificate would be coded as follows:

This document defines industry and purpose codes currently being used. The purpose codes are listed alphabetically by title with definitions and codes, and alphabetically by code with titles.

Even though the year and sequence codes offer unique case identification, the industry and purpose codes should also be used on all case documents to facilitate processing. CIVIL FORFEITURE (CVF) - Application to hold hearings relative to Civil Forfeiture assessments for motor carrier safety and economic violations.

COMMISSION INITIATED COMPLAINT (COC) - A complaint brought by this Commission concerning some specific action taken by a public utility or transportation company.

COMMISSION INQUIRY (COI) - A formal inquiry into any matter under the jurisdiction of this Commission.

COLLABORATIVE PROCESS (COL) - A Commission proceeding in which a collaborative process is used to resolve issues arising from a utility application.

COMPETITIVE/ALTERNATIVE TELECOMMUNICATION SERVICES (COM) - An application filed by a large local exchange telephone company pursuant to Section 4927.03, Revised Code.

COMPLAINT ON OPERATING AUTHORITY (CRA) - A complaint alleging misuse of operating authority granted by this Commission.

COMPLAINT ON A RAILROAD FOR DAMAGE CLAIMS (CDC) - A complaint filed against a railroad for property damage, delays, or over- charges upon goods shipped by rail (Section 4907.57 O.R.C.).

COMPLAINT ON MUNICIPAL RATE (CMR)

COMPLAINT ON RATES CHARGED (CRC) - A complaint filed by a consumer alleging improper charging of rates by any regulated company except a railroad.

APPLICATION TO PASS-THROUGH COAL RESEARCH AND DEVELOPMENT COSTS (CRD) - An application by gas or natural gas companies to pass-through Coal Research and Development Costs to customers, pursuant to 4901:1-12, O.A.C.

COMPLAINT ON SERVICE OR SAFETY (CSS) - A complaint initiated by a consumer concerning the service rendered by a regulated company or the safety of its operations.

DECERTIFICATION COST RECOVERY (DCR) Request for decertification cost recovery rider.

ELECTRIC FUEL COMPONENT (EFC) - A Commission review of the Electric Fuel Component contained within the rate schedules of a regulated electric utility company and related matters.

COMMISSION PROCEDURE FOR THE RECOVERY OF EMISSION FEES (EFR) - The Commission procedure for the recovery of emission fees levied upon an electric light company.

EMERGENCY 911 SERVICE RATES (EMG) - An application by a telephone company for approval of a tariff to establish rules and rates for 911 emergency service.

ENVIRONMENTAL COMPLIANCE PLAN (ECP) - A plan developed by an elective company to comply with the acid rain control requirements at its generating facilities that are affected by the Phase I acid rain control requirements under Title IV of the Clean Air Act Requirements of 1990. (Chapter 4913 O.R.C. pursuant to S.B. 143).

FUEL ADJUSTMENT CLAUSE REVIEW (FAC) - A Commission review of the fuel adjustment clause contained within the rate schedules of a regulated electric utility company.

FEDERAL ACTIVITIES DOCKET (FAD) - A Commission proceeding established for Commission comments in federal matters for each utility type.

FEDERALLY FUNDED WARNING PROJECTS(FED) - A Commission proceeding involving the implementation of federally funded warning projects for railroad crossings.

GAS ADJUSTMENT CLAUSE (GCR) - A Commission review of the Purchased Gas Adjustment Clause contained within the rate schedules of a regulated gas utility company and related matters.

GAS PIPELINE SAFETY (GPS) - A Commission investigation into the compliance with Natural Gas Pipeline Safety Standards and related matters (4901:1-13-09 through 4901:1-13-20 OAC).

MINIMUM LEVEL FLEXIBLE PRICING (FLX) - Flexible Rate changes - notifications of rate changes and Tariff filings pursuant to flexible rate authority.

PUBLIC WAY REGULATORY ASSETT (PWR) - An application by a public utility to create a regulatory asset for the purpose of recovering a public way fee.

PUBLIC WAY TARIFF (PWT) - An application by a public utility to revise its tariff to provide for the recovery of a public way fee.

PURCHASED WATER ADJUSTMENT (PWA) - A Commission review of the purchased water adjustment contained within the rate schedules of a regulated water utility and related matters.
