Annual Report Filing Instructions:
The filing deadline for Annual Reports for the 2012 calendar year was extended to May 30, 2013, pursuant to the Attorney Examiner’s entry in Case No. 13-01-AU-RPT issued April 29, 2013.
The PUCO has developed a new on-line filing system for all industry segments using a uniform core report and affidavit for all industry segments, that does not require you to file the report in a case docket or send a copy to the Ohio Consumers Counsel. As with all new systems, we expect some issues and appreciate your cooperation and feedback in making this process as efficient as possible for all concerned. Please contact us at (614) 466-4095 or send a message to our annual reports mailbox. Please do not submit old forms.
If you electronically filed your report last year, the new system will work with your same PUCO username and password, but you will need to request access for each company (and certificate) on whose behalf you will be filing. As was the case last year, we are requiring a report to be filed for each certificate or authority held.
A new Affidavit form has been adopted for all annual reports. The affidavit must be signed by a company officer authorized to file the report, and duly notarized. The form adds a new section that allows the officer to designate the employee, accountant or third-party contractor, who will make the actual filing on behalf of your company. Here's a sample affidavit in JPG format, showing the typical situation of a company president signing a report that will be filed by a third-party accountant. Please note that only the company officer's signature needs to be notarized. The employee, accountant or third-party contractor can complete the bottom section, if applicable, after officer's signature is notarized. A scanned image or picture of the completed affidavit (in JPG, PNG or BMP format) must be uploaded as part of the new on-line filing process for all industry segments.
In addition to the uniform core report and affidavit, some industry segments (see the chart below) will be required to upload supplemental schedules as part of the filing process.
Listserves & Supplemental Schedules:
All regulated entities should subscribe to the appropriate industry list(s) by going to the Industry Listserve Sign Up, and selecting the appropriate industry service list(s) as shown in the chart below. The use of company mailboxes and multiple addresses should be considered to facilitate e-mail service, and companies should take appropriate action to ensure that electronic mail service from the Commission is not blocked or disabled.
