Annual Report Filing Instructions

NEW FOR THIS YEAR:

Any regulated entity that wishes to continue receiving paper copy service from the Commission’s Docketing Division must submit a Request for Paper Service form.
All regulated entities will be subject to a $100 minimum assessment regardless of intrastate gross revenues, pursuant to Ohio Revised Code Section 4905.10(A).

All regulated entities can subscribe to the appropriate industry list(s) by going to the Industry Listserve Sign Up, and selecting the appropriate industry service list(s) as shown in the chart above. Companies should subscribe multiple addresses and take appropriate actions to ensure that electronic mail service from the Commission is not blocked or disabled.
