Annual Report Filing Instructions:
The filing deadline for Annual Reports for the 2013 calendar year is April 30, 2014, pursuant to the Commission's January 22, 2014 order in Case No. 14-01-AU-RPT.
Notice and filing instructions will be sent to all filing, docketing & fiscal contacts as soon as the on-line system is ready. Please note that a separate report should be filed for each certificate or authority held.
A standardized Affidavit form has been adopted for all annual reports. The affidavit must be signed by a company officer authorized to file the report, and duly notarized. The form adds a new section that allows the officer to designate the employee, accountant or third-party contractor, who will make the actual filing on behalf of your company. Here's a sample affidavit in JPG format, showing the typical situation of a company president signing a report that will be filed by a third-party accountant. Please note that only the company officer's signature needs to be notarized. The employee, accountant or third-party contractor can complete the bottom section, if applicable, after officer's signature is notarized. A scanned image or picture of the completed affidavit (in JPG format) must be uploaded as part of the new on-line filing process for all industry segments.
In addition to the uniform core report and affidavit, some industry segments (see the chart below) will be required to upload supplemental schedules as part of the filing process.
Listserves & Supplemental Schedules:
All regulated entities should subscribe to the appropriate industry list(s) by going to the Industry Listserve Sign Up, and selecting the appropriate industry service list(s) as shown in the chart below. The use of company mailboxes and multiple addresses should be considered to facilitate e-mail service, and companies should take appropriate action to ensure that electronic mail service from the Commission is not blocked or disabled.
