In lieu of mailing a paper copy for completion, telephone (ILEC) companies should download annual report forms from the Commission's website at http://www.puco.ohio.gov/puco/forms/. The telephone electronic annual reports forms are Microsoft Access database files which can be completed using Microsoft Access 2000, Microsoft Access 2003 or Microsoft Access XP. If the company does not have any of these versions of Microsoft Access, the company may download and use a "runtime" version of the program and files without purchasing any software. If the company wishes to receive the files or "runtime" version on CD or otherwise requires technical assistance, please request an email contact at the Commission. Print commands are built into the files to enable the filer to print a hard copy of the annual report. Upon completion of the electronic form, the company should print a hard copy, obtain all required signatures, and file then hard copy with the Commission and Ohio Consumers' Counsel no later than April 30, 2007, by sending copies to:

The company should also electronically file its report by April 30, 2007, at https://www.puc.state.oh.us/secure/garupload/index.cfm. Please note that the company will first need to request an email contact in order to obtain a username and password, and be granted file uploading privileges.
If you have Microsoft Access 2000, Microsoft Access 2003, or Microsoft Access XP, right click on the name of the file that corresponds your version and then select "SAVE AS" to save the file to your local hard drive. Caution: enter data and save your file to local on your hard drive.

If you do not have Microsoft Access, click on AnnualRptTelephone2006.zip (124 MB), extract the zip file, and run "setup.exe". This will install the Runtime version of Access on your computer and AnnualRptTelephone2006.mdb and other necessary files in the directory C:\AnnualReportGas. A shortcut to the file will be installed under Start, Programs. More detailed directions are as follows:
1. Go to Start and Run D:\AnnualRpTelephone2006.exe (D = your
CD Rom Drive ) click OK.2. At the message entitled, “WinZip Self-Extractor”, click UNZIP. The second time that “WinZip Self-Extractor” appears, the files should have been unzipped successfully, therefore, you can click CLOSE.
3. Next, open your C: drive, and go to Annual Reports and double click on setup.exe to open. Follow along with the additional instructions on the messages as they appear.
4. When the message appears to reboot your computer click YES (the install process may ask you to reboot a couple of times).

NOTE: The AnnualRptTelephone2006.exe file may take a long time to download. If you find that this is the case, we can send this version to you on a CD. Please contact us at the number below so that a CD can be mailed out to you.
If your computer has restrictions on installing software, this program either will not install at all or it may install improperly. If this is the case, contact your System Administrator.
You may need to install an authorized copy of winzip in order to run the AnnualRptTelephone2006.exe program, please contact your system administrator.
When you uninstall a run-time application (AnnualRptTelephone2006.exe program) by using the REMOVE ALL option, you may get the following error: "Can't remove 32autole.dll, this file is in use by another application." If you get this error message, click IGNORE and all other files will be removed. Caution: you may not want to uninstall this program if you want to view your electronic annual report database in the future.
After uninstalling Microsoft Access, users of the run-time application may find that .mdb files no longer automatically open in Microsoft Access even though the run-time version of Microsoft Access is still installed on the machine. To work around this problem, users can either reinstall the application or manually add an association between .mdb and MSaccess.exe.
Here are INSTRUCTIONS on COMPLETING the Annual Report Database.
When the electronic annual report database has been completed:
1. Print a hard copy,
2. With the appropriate and required signatures, sign the hard copy,
3. Docket the signed hard copy pursuant to the Commission's Entry in Case No. 04-326-AU-UNC,
4. To submit your completed electronic annual report file contact the Commission by email to receive transmission instructions and to obtain file uploading privileges.
