If you work for a motor carrier and have safety concerns, you may file a complaint with the PUCO by completing then printing and mailing or faxing this form to:

The PUCO does not investigate complaints regarding economic issues unless they are household goods-related. If the complaint involves interstate freight, the complaint should be filed with Federal Motor Carrier Safety Administration (FMCSA).

As a result of handling your complaint, all documents that you send to the PUCO are subject to the Ohio Public Records Act. This complaint form and all attached documentation will be considered public record and will be available for inspection by anyone, upon request.
