Public Utilities Commission Annual Reports - 2008 Instructions for filing

2008 Annual Report Filing Instructions

NOTE: There are separate instructions for natural gas distribution companies and local exchange telephone companies.

The PUCO is making several changes in the processing of annual reports for 2008. First, all regulated entities are directed to subscribe to a new industry listserve by selecting the Docketing link on the PUCO Home page (www.puco.ohio.gov). Then click on the Industry Listserve Sign Up link and subscribe to the appropriate industry service list:

The Commission is exploring the use of this method of e-mail service to increase efficiency and reduce postage expenses in the future. Companies should subscribe multiple addresses and take appropriate actions to ensure that electronic mail service from the Commission is not blocked or disabled. 

Second, the Commission is requesting all companies to make parallel filings of their annual reports both by paper and electronically by filing the completed report using the PUCO’s Docketing Information System (DIS). 

Filers who already have a PUCO account (a username, password, and E-filing profile) should file the PDF (portable document format) version of the completed report in Case No. 09-1-AU-UNC.  The filer should also upload any source documents used to create the PDF.  A scanned image should be provided for notarized signatures. 

Filers who do not already have a PUCO account should first review the electronic filing manual posted under the electronic filing link at www.puco.ohio.gov/puco/docketing/. In order to participate in the e-filing program, you will need to submit an electronic filing participation agreement and create a PUCO account (obtain a username and password) by going to www.puco.ohio.gov/puco/docketing/ and clicking on Get a NEW PUCO Account. After creating an account, you will be able to log in to DIS and create an e-filing profile. (Please see steps 3 and 4 in the electronic filing manual for further details.)

Each regulated entity is directed to E-file its annual report in Case No. 09-1-AU-UNC by April 30, 2009. (Please see the electronic filing manual for further details on the filing process.)  If you are unable to successfully complete this process, or wish to submit comments or suggestions regarding the e-filing process or electronic service, please file your comments by paper or electronically in Case No. 09-1-AU-UNC by April 30, 2009. 

Each regulated entity must also submit paper copies no later than April 30, 2009 to:
